Dr. Grobman Provides Dementia Care Under
Medicare’s GUIDE Model

Call us at 302-329-8452 to learn more and answer your questions.

Dementia is a difficult disease to manage, whether you are a patient or a caregiver. This is why Medicare has started the GUIDE program with no copays to help dementia patients and their caregivers. In addition, Medicare will cover $2500 of respite care annually to qualifying caregivers.

I am encouraging all my dementia patients to enroll in this program and will also provide GUIDE care to patients outside my primary care practice. GUIDE allows us to have a dedicated Care Navigator work regularly with you and your caregiver virtually to address your specific needs, answer your questions, and coordinate your care to help keep you safe in your home.

Guiding an Improved Dementia Experience (GUIDE) Model Goals:
