Dementia is a difficult disease to manage, whether you are a patient or a caregiver. This is why Medicare has started the GUIDE program with no copays to help dementia patients and their caregivers. In addition, Medicare will cover $2500 of respite care annually to qualifying caregivers.
I am encouraging all my dementia patients to enroll in this program and will also provide GUIDE care to patients outside my primary care practice. GUIDE allows us to have a dedicated Care Navigator work regularly with you and your caregiver virtually to address your specific needs, answer your questions, and coordinate your care to help keep you safe in your home.

GUIDE Care Delivery Services Include:
Personalized Care Plans
Comprehensive assessment for each patient forms the basis for a personalized care plan.
Caregiver Respite Services
Qualifying caregivers are supported with temporary respite services of $2,500 per year paid by Medicare.
Medication Management and Reconciliation
Ongoing Monitoring and Support
Care Navigators provide guidance via frequent telehealth visits in the comfort of your home.
Care Team Coordination
Care team coordination improves patient care and reduces caregiver burden.
Ensures the right medications are taken at the right times.
All GUIDE services are provided with no cost or copayments for qualifying Medicare beneficiaries.
Eligibility for participation in GUIDE is limited to beneficiaries with diagnosed dementia who have Traditional Medicare as their primary insurance. At this time, GUIDE is not available for patients with Medicare Advantage, those residing in a skilled nursing facility, or patients enrolled in PACE or hospice programs.
